Mamata Hospital (“we,” “us,” or “the Hospital”) respects the privacy of patients, families, website visitors, and newsletter subscribers. This Privacy Policy explains what information we collect through mamatahospital.com, why we collect it, how long we keep it, with whom it may be shared, and the choices you have. It is written for a United States audience and is intended to align with the spirit of the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule for protected health information (“PHI”) created in a care setting, as well as general consumer privacy expectations. This website copy is educational; a live hospital implementation should be reviewed by privacy counsel before production use.
This Policy covers the public website, appointment-request forms, contact forms, blog comment fields, and subscribe/unsubscribe pages. It does not replace the Notice of Privacy Practices you receive when you become a patient in a clinical setting. If there is a conflict between this website policy and a signed clinical notice, the clinical notice controls for PHI held in the designated record set.
This static version of the Site performs validation in your browser only and does not send form data to a server. The commitments below describe how Mamata Hospital intends to treat personal information when online tools are connected to operational systems.
When you browse, standard web logs may record browser type, language, referring pages, timestamps, and approximate region derived from IP address. We may use cookies or similar technologies to remember a language preference, keep a search overlay state, or understand which articles are most read. We do not use website forms to collect Social Security numbers, full payment-card numbers, or genetic data, and you should not paste those values into message boxes.
If you describe symptoms, medications, or diagnoses in a form, that content may be PHI once it is received by Hospital workforce members. Please share only what is needed to route your request. Do not include another person’s medical information unless you are that person’s authorized personal representative.
We use personal information to:
We do not sell personal information. We do not use website form contents to train public generative-AI models.
For website visitors who are not yet patients, we process information based on your request for information or services and our legitimate interest in operating a secure hospital website. Once information is used for treatment, payment, or healthcare operations, HIPAA and state medical-privacy laws apply. We will not use or disclose PHI for marketing that requires authorization without that authorization, and we will not sell PHI.
State laws may provide additional rights (for example, consumer health-data rules in certain states). Where state law is more protective than this Policy, we follow the more protective rule for residents of that state when we have a legal duty to do so.
We may share information with:
We do not share newsletter lists with unrelated advertisers. Social-media icons on the Site are links you choose to follow; those platforms have their own policies.
Appointment and contact records that become part of a medical chart are retained according to medical-record retention schedules (often years, and longer for minors). Newsletter emails are kept until you unsubscribe or the list is retired. Website server logs, if used, are typically rotated on a shorter cycle unless needed for security investigation. When we no longer need information, we delete or de-identify it in accordance with Hospital policy.
We implement administrative, technical, and physical safeguards appropriate to a hospital environment, including access controls for workforce members, encryption in transit for live form submissions, and staff training on minimum necessary use. No method of internet transmission is perfectly secure. You can reduce risk by using a private device, locking your screen, and avoiding public Wi-Fi when sending health details.
Essential cookies may support page functionality. If we enable analytics, we will configure them to avoid collecting unnecessary identifiers where practical. You can control cookies through your browser settings; blocking all cookies may affect search overlays or form convenience features.
The Site is intended for adults and for parents submitting on behalf of children. We do not knowingly collect personal information directly from children under 13 through website forms. If you believe a child submitted information, contact us through the Contact page so we can delete it from website systems.
You may:
We will not retaliate against you for filing a privacy complaint. Some requests may be denied when another law requires us to keep a record, when information is part of psychotherapy notes with special protections, or when disclosure could endanger someone.
The Site is operated for a U.S. audience. If you access it from another country, you understand that information may be processed in the United States, where privacy laws may differ from those in your home country.
Embedded fonts, icon libraries, and optional video players are delivered by third parties. Their collection of technical data is governed by their policies. Review those policies if you are concerned about cross-site tracking.
If a breach of unsecured PHI occurs, we will provide notifications required by HIPAA and state law. For non-PHI website incidents, we will take reasonable steps to contain the issue and inform affected users when legally required or when we believe notification would reduce harm.
We may update this Policy to reflect new services, laws, or technologies. The “Last updated” date will change. Material changes will be posted on this page. Your continued use of the Site after posting means you acknowledge the revised Policy.
For privacy questions, records requests, or to exercise a right described above, use the Contact Us form and select a message that includes the word “Privacy.” Do not include full medical histories in the first message; a representative will tell you the secure method for follow-up. You may also review our Terms & Services for website use rules.
