These Terms & Services (“Terms”) govern your access to mamatahospital.com (the “Site”) and your use of informational content, appointment-request tools, newsletter features, and related online services offered by Mamata Hospital (“we,” “us,” or “the Hospital”). By browsing or submitting information through the Site, you agree to these Terms. If you do not agree, please discontinue use.
The Site is an educational and administrative resource for patients, families, and referring clinicians in the United States. Articles, department descriptions, physician biographies, and statistics describe our programs in general terms. They are not a diagnosis, treatment order, or guarantee of a particular clinical outcome. Only a licensed clinician who has evaluated you can provide medical advice specific to your condition.
Nothing on the Site creates a physician–patient relationship by itself. Submitting an appointment request, comment, or contact form is an invitation for the Hospital to respond; it does not reserve a procedure slot, admit you as an inpatient, or replace emergency evaluation when symptoms are time-critical.
You must be at least 18 years of age to submit forms on your own behalf. Parents or legal guardians may submit requests for minors. You agree that information you provide is accurate to the best of your knowledge. You will not impersonate another person, submit another patient’s protected health information without authority, or use automated scripts to overload the Site.
Online forms on this Site operate with client-side validation only. They help you complete required fields; they do not transmit data to a hospital server in this static demonstration. In a live environment, submitted details would be reviewed by scheduling or patient-support staff. You remain responsible for attending confirmed visits, bringing identification, and updating medication lists.
We may decline, reschedule, or redirect a request when the selected department is not appropriate, when information is incomplete, or when urgent symptoms require a different care pathway. Repeated abusive or fraudulent submissions may result in blocking further form use.
The Site is not an emergency dispatch system. If you believe you or someone else is experiencing a life-threatening emergency, seek immediate in-person emergency care through local emergency services. After you are safe, you may use the Site to request follow-up with a Mamata Hospital department.
We strive to keep service descriptions current, but protocols, hours, and clinician availability change. Educational blog posts summarize common public-health guidance and may not reflect the newest trial data. The Mamata Hospital name, logo, layout, original text, and original graphics are owned by the Hospital or its licensors. You may view and print pages for personal, non-commercial use. You may not copy the Site for a competing hospital website, scrape content at scale, remove notices, or use our marks in a way that suggests endorsement without written permission.
Comments you post grant the Hospital a non-exclusive, royalty-free license to display, moderate, or remove that content in connection with operating the Site. You represent that your comments do not infringe others’ rights and do not contain another patient’s confidential medical details.

TO THE MAXIMUM EXTENT PERMITTED BY LAW, MAMATA HOSPITAL AND ITS CLINICIANS, EMPLOYEES, AND CONTRACTORS SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, DATA, OR GOODWILL, ARISING FROM YOUR USE OF THE SITE. OUR TOTAL LIABILITY FOR CLAIMS ARISING OUT OF THE SITE SHALL NOT EXCEED ONE HUNDRED U.S. DOLLARS (USD $100). THIS LIMITATION DOES NOT AFFECT LIABILITY THAT CANNOT BE LIMITED UNDER APPLICABLE LAW, INCLUDING LIABILITY FOR GROSS NEGLIGENCE OR WILLFUL MISCONDUCT WHERE SUCH EXCLUSION IS PROHIBITED, OR LIABILITY ARISING FROM IN-PERSON CLINICAL CARE GOVERNED BY SEPARATE INFORMED-CONSENT AND MALPRACTICE RULES.
You agree to indemnify and hold harmless Mamata Hospital from claims, damages, and reasonable attorneys’ fees arising from your misuse of the Site, your violation of these Terms, or your infringement of another person’s rights, except to the extent caused by our willful misconduct.
Personal information submitted through forms is described in our Privacy Policy. Please read it before sending health-related details. Newsletter subscription and unsubscription are handled through dedicated pages in the Site footer.
We may update department descriptions, clinician listings, hours, and these Terms at any time. The “Last updated” date will change when we do. Continued use after posting constitutes acceptance of the revised Terms. Material changes to how we handle personal information will also be reflected in the Privacy Policy.
These Terms are governed by the laws of the United States and the state in which the relevant Mamata Hospital facility providing your in-person care is located, without regard to conflict-of-law rules. Informal resolution is encouraged: contact us through the Site before filing a claim. Any dispute relating solely to website use shall be brought in a court of competent jurisdiction in the United States. These Terms do not limit any non-waivable consumer rights you may have under state law.
If a provision is held unenforceable, the remaining provisions continue in effect. These Terms, together with the Privacy Policy, constitute the entire agreement regarding Site use and supersede prior online notices on the same subject. Failure to enforce a provision is not a waiver.
